Client Info for Wellness Packages 

 

How will a wellness package benefit me and my pet? 

  • One fee for all yearly routine services rendered.

  • Dogs:  Free year of Heartgard

  • Cats:  Two free bags of food

  • Discounted price as packaged deal.

Client Instructions:  

  • Adult pets should complete two visits.

               1.  Exam, diagnostic testing

               2.  Dental, vaccines

  • Kittens/Puppies should complete three to four visits as outlined.

              (For kittens, there are packages available with and without a declawing procedure.)

  • You will be given a handout describing all services included in your pet’s specific wellness package.  You may keep track of the procedures completed throughout the year, but we will always keep a computerized and hard-copy so you don’t have to.

  • Packages will expire one year after purchase for adult pets, and at six months of age for puppies and kittens.

  • Please bring in a fecal sample to the visit specified.

  • On you adult pet’s first visit, please do not let your pet urinate just prior to coming in.  We may need to collect a urine sample.

  • On your pet’s last visit, please do not offer food after midnight the night before, as your pet will be going under general anesthesia.

  • This is a well-pet package.  All additional services and products (antibiotics, extractions, dewormers, etc.), including those associated with illness and injury during the package term, will be charged as usual.

  • Packages are non-transferable and non-refundable.

What should I know about the free items discussed?

  •               In an effort to continue the all-inclusive care you have chosen for your pet, the following will be offered at no charge.

               1.  Dogs:  twelve months of Heartgard (up to 100 lb dose).

               2. Cats:   two large bags of food (you can choose to take the second

                   one now if you wish, or at another visit.
